All new appointments require a 50% deposit, effective July 1, 2026. This deposit will be deducted from the cost of the initial evaluation. This deposit is nonrefundable if appointments are not cancelled or rescheduled one business week prior to the appointment. A business week for the Sports Med/Rehab service is Monday through Thursday, 7:30 a.m. to 5:00 p.m.
